About NY FACE

  • NY FACE is a NYS Department of Healthworkplace health and safety program. Its purpose is to study work-related fatal injuries in New York State to determine how the injury occurred.
  • NY FACE is about prevention. It provides educational materials to raise awareness among employers and workers of commonly occurring hazards and injuryprevention practices.
  • NY FACE is not an enforcement program and participation is voluntary. It does not check to see if the employer is in compliance with Occupational Safety and Health Administration (OSHA) standards.
  • NY FACE is not interested in “who to blame” but rather in “what went wrong” and how a similar death can be prevented in the future.
  • NY FACE does not use the names of the deceased, co-workers, employers, or incident site owners in its publications.

NY FACE Investigations

The employer's role

An employer should report a work-related death by contacting the NY FACE program in a timely manner. Employers can provide important information regarding the incident that led to the death of the worker. Employers are asked to provide information about their health and safety programs and assist NY FACE investigators in visiting the incident site and speaking with witnesses. A site visit is necessary for investigators to collect information about what was happening just before, at the time of, and right after the fatal injury.

How employers benefit

The employer receives a free, impartial assessment of the workplace that helps identify factors that may have contributed to a worker's death. This knowledge can help prevent future injuries.

How NY FACE information is used

The information that is collected is used to identify causes and detect common patterns of workplace deaths. Information summarizing the on-site investigation with recommendations to prevent similar fatal incidents is distributed to employers, concerned individuals, and safety organizations.
