Approved Early Intervention Health Home Providers
Early Intervention (EI) is a statewide program that provides many different types of therapeutic and supportive services to infants and toddlers, birth to three years of age, with disabilities and their families. It is the mission of the Early Intervention Program to identify and evaluate as early as possible those infants and toddlers whose healthy development is compromised and to provide for appropriate intervention to improve child and family development.

The NYS DOH Health Home program and the Bureau of Early Intervention is pleased to announce the following agencies have been approved to enroll Early Intervention children within the Health Home program and the lead Health Home(s) they will be working with due to being cross-trained to serve both EI and HH eligible children.
