Suggested SADC Site Evaluation Tool for MLTC Plans
Description: The Suggested SADC Site Evaluation tool is an example tool for MLTC plans to utilize when performing annual SADC site assessments. If an MLTC plan intends to utilize their own site evaluation tool, it must contain all components from the DOH suggested tool. The suggested tool guide is a companion to the suggested tool which, along with the video tutorials, provides additional information related to the use of the suggested tool and guide.
