Linking Interventions For Total Population Health (LIFT Population Health)
Background
LIFT is a population health initiative that funds communities to develop a comprehensive, prevention-focused approach to address one chosen health issue by linking together primary care, public health and community partners and resources. Funding for this initiative is through the State Innovation Model award provided by the Center for Medicare and Medicaid Innovation.
Purpose
The purpose of the LIFT project is to support activities that help New York State achieve its goals of improving health and health care as described in the State's Prevention Agenda and the State Innovation Model initiative. Grantees will develop and implement a coordinated portfolio of aligned and mutually reinforcing interventions that span activities included in each of the three buckets of prevention:
- Bucket 1: Traditional clinical preventive interventions
- Bucket 2: Innovative preventive interventions that extend care outside the clinical setting
- Bucket 3: Total population or community-wide interventions

Priority populations are those population subgroups with prediabetes and uncontrolled hypertension who are at risk for developing Type 2 diabetes and cardiovascular disease. Grantees will support prevention activities that align with and leverage existing health system redesign in target communities.
