Goals:

  • Create community environments that promote and support healthy food and beverage choices and physical activity
  • Prevent childhood obesity through early child-care and schools
  • Prevent initiation of tobacco use by New York youth and young adults, especially among low socioeconomic status (SES) populations
  • Promote tobacco use cessation, especially among low SES populations and those with poor mental health
  • Eliminate exposure to secondhand smoke
  • Prevent suicides among youth and adults

Interventions:

  • Increasing adoption and use of food standards
  • Promoting of policies and practices in support of breastfeeding
  • Adopting tobacco-free outdoor policies
  • Establish linkages with the OMH Early Recognition and Screening Initiative in the region including the creation and promotion of a suicide prevention mobile phone app
  • Build community coalitions that advance the State's 'Suicide as a Never Event' through promotion and prevention activities
  • Develop a public education campaign and ongoing online campaign on means restrictions centered on the safe and secure storage of firearms and the safe disposal of unwanted prescription medications

  • Reduce Illness, Disability and Death Related to Tobacco Use and Secondhand Smoke Exposure.

  • Promote Mental, Emotional and Behavioral Well-Being in Communities.

Goals:

  • Increase participation of adults with diabetes in a course or class to learn how to manage their condition
  • Promote mental, emotional and behavioral (MEB) well-being in communities

Interventions:

  • Collaboration with a Federally Qualified Health Center on our campus to refer diabetic or pre-diabetic patients from our Emergency Department to their Diabetes Self-Management programs and to their team of Diabetes Care providers
  • Collaborate with FHC on an integrated approach to identify and provide affordable short term evidence-based treatments to patients seeking individual treatment for depressive and other mental health disorders

  • Increasing the availability, accessibility and use of evidence-based interventions in self-care management in clinical and community settings
  • HAHV provides hospital program called "Practical Tips for Promoting and Maintaining MED Wellness in Youth"--focused on depression, anxiety and suicide prevention
  • Margaretville site to offer ASIST (Applied Suicide Intervention Skills Training)
