Goals:

  • Increase screening rates for cardiovascular diseases; diabetes; and breast, cervical and colorectal cancers, especially among populations experiencing health disparities
  • Increase percentage of adults whose Hemoglobin A1C is <8% and promote culturally relevant chronic disease self-management education
  • Increase the percentage of adults with access to a primary care provider
  • Increase the percentage of adults seeing dentist in the last year
  • Increase the number of babies that are breastfed in New York State

Interventions:

  • Certify and Maintain Diabetic Management Program at AO Fox Hospital and offer to all patients
  • Increase referrals to Diabetic Management Program through evaluating Hemoglobin A1C screening during primary care appointments and collaborating and educating Network affiliates about the program
  • Certify and Maintain half of the Primary Care Clinics through National Committee for Quality Assurance (NCQA) Diabetic Recognition Program
  • Recruitment and placement of additional Primary Care Providers in Oneonta and Sidney
  • Participation in Regional Primary Care Plan Development
  • AO Fox continues to support the Oneonta Community Health Center which is a free clinic for the uninsured and those residents who do not qualify for health care insurance by providing free lab and x-ray services
  • Achieve 100% attestation rate for meaningful use stage 2 measures for all primary and specialty care providers
  • Use media and health communication to build public awareness and demand
  • Build education for breast screening, colorectal screening, cardiovascular disease, and diabetes into every Primary Care Visit using evidence based guidelines to determine what population will receive which education during Primary Care Visits (ie, Wo men over 40 will automatically receive education pertaining to breast cancer screenings; All adults 50-75 will be offered colorectal cancer screenings)
  • Collaborate with the Network affiliates to support use of alternative locations to deliver preventative services and educate patients on those alternative options
  • Collaborate with Population Health Management vendors to provide reminder calls and letters to patients in the regarding overdue or upcoming screenings and appointments
  • Increase collaboration among dental health professionals in the Bassett Healthcare Network through participation in the Otsego County Oral Health Coalition and hosting annual Dental Network Retreats
  • Improve community awareness of the FoxCare Center Dental facility using a variety of advertising methods
  • Provide Dental Services to the uninsured and underinsure through partnering with Medicaid insurance plans and working with the Oneonta Community Health Center serving the uninsured population with dental care
  • Recruit and retain 1 full-time general dentist, 1 part-time periodontist, 1 full-time general/peds dentist
  • Develop and expand the dental health workforce through working with Columbia to explore a Dental Residency Program at AO Fox Hospital
  • Participate in New York State Breastfeeding Quality Improvement Hospital Initiative
  • Collaborate with MCAT Mobile Crisis Assessment Team
  • Provide Primary Care Providers with training regarding use of MCAT in the primary care setting
  • Leverage EMR system to include assessment of MEB disorders and substance abuse during primary care visits

  • Conduct Continuing Medical Education (CME) programs for health professionals, including programs on diet, exercise, stress, coping, obesity and disabilities
  • Support use of alternative locations to deliver preventive services, including cancer screening Mobile Coach
