Social Care Networks

The State aims to build a more resilient, flexible, and accessible social care delivery system that reduces health disparities and advances health equity, as outlined in the State's 1115 Waiver Demonstration Amendment. The State has moved to establish regionally based Social Care Networks (SCNs) to increase and strengthen the delivery of social care services to Medicaid members. These SCN entities will coordinate the delivery of social care services to Medicaid members by community-based organizations (CBOs), with support from shared data and technology. SCN lead entities will be charged with building reliable networks of contracted Social Service Providers to better enable HRSN screening and the delivery of social care services.

The SCNs will impact the future state system for Medicaid members across four core dimensions:

  • Scaled delivery of social care services and improved access for Medicaid Members
  • Reliable and timely referral of members to social care services
  • Seamless tracking of members needs to streamline and close loop on referrals to social care services
  • Improved and increased collaboration between social care services and other partners in regional ecosystem (e.g., healthcare providers, care management providers, Managed Care Organizations, others)
