Medically Fragile Children's Program

Enrollment

The Medically Fragile Children´s Private Duty Nursing Program consists of two components:

  1. Private Duty Nursing (PDN) Medically Fragile Children (MFC) Training and Experience
  2. PDN MFC Provider Directory

Both components have corresponding financial enhancements for provider participation, and require provider enrollment. A provider can choose to enroll in one or both programs.  Re-enrollment into each component will be necessary during a the Provider Enrollment Revalidation Process, which occurs every five years.

Providers can enroll by completing the appropriate form for the PDN Program for MFC, available below:

Completed enrollment forms can be submitted in any of the following ways:

  • Mail: eMedNY, PO Box 4610, Rensselaer, NY 12144-4610
    • Forms must be mailed if submitted during the initial Medicaid Provider Enrollment Process
    • Submission via mail is also available to current Medicaid providers
  • Fax: 518-473-7251
    • Coversheet must include "Medically Fragile/PDN Directory Update" and the name and contact number of the sender
    • Submission via fax is only available to current Medicaid providers
