Review of Service definitions - David Hoffman

  1. Service Coordination (SC) and Community Integration Counseling (CIC) were presented as an integrated service at the prior workgroup meeting, as Community Integration Services. The services have been restored as two separate services. Key discussion points regarding SC and CIC:
    1. Defining the roles and responsibilities of the service coordinator as compared to the care manager at the Managed Care Organization (MCO).
    2. Developing trainings to educate those in the field, of both the roles of service coordinator and care manager roles.
    3. Discussion of the timeline for developing model contract language.
    4. Defining the assessment tool and the assessment process that would be utilized to determine the need for SC.
    5. Discussion of continuity of care and contracting with current waiver service providers after the transition.
    6. Request to add provider qualifications to the agenda for the next meeting.
    7. Request to add fiscal implications and analysis of provider contracting to the agenda for future meetings.
    8. Discussion of the provision of CIC in a group setting.
    9. Clarification that CIC may be included as a CFCO service based on the individual's needs and the method of service provision.
  2. Independent Living Skills Training (ILST)
    1. Review of service definition as provided in the updated crosswalk.
    2. Defining the qualifications of an ILST provider.
    3. Discussion of the roles of the ILST provider in relation to the MCO Care Manager.
    4. Suggestion to add a required initial assessment of the member to the definition of ILST with ongoing assessments as needed for reevaluation.
  3. Positive Behavioral Interventions and Support Services (PBIS)
    1. Review of service definition as provided in the updated crosswalk.
    2. Defining the qualifications of a PBIS provider.
    3. Discussion of the assessment/reassessment schedules and reimbursement rates.
  4. Structured Day Program (SDP)
    1. Review of service definition as provided in the updated crosswalk.
    2. Discussion of SDPs and the federal Home and Community Based Services Settings regulations.
    3. Suggestion that SDP activities be extended to include weekend activities.
  5. Home and Community Support Services (HCSS)
    1. Review of service definition as provided in CFCO State Plan Amendment (SPA) and updated crosswalk.
    2. Discussion of billing procedures and reimbursement for HCSS staff training.
    3. Concern regarding the inclusion of HCSS under the CFCO umbrella rather than a discrete service offered through the proposed RRDC model.
    4. Discussion of oversight and supervision. Requests to include HCSS as a discrete service.
  6. Additional discussion
    1. Discussion of the transition timeline.
    2. Discussion of required training for managed care plans to work with this population and the new services.
    3. Discussion of continued public input throughout the transition process and including waiver participants in the development training and education opportunities.
