Social Determinants of Health and Community Based Organizations

Goal: The Social Determinants of Health goal is to formulate and provide specific recommendations that drive VBP by addressing social determinants of health. The Community Based Organizations (CBOs) goal is to formulate and address the training needs for CBOs and ensure all pertinent organizations are involved.
