DSRIP PDG Midpoint Deliverables

On October 2, 2014, the NYS Department of Health sent a Midpoint Assessment questionnaire to all the lead Performing Provider Systems (PPSs). The DSRIP PDG Midpoint deliverables questionnaire was due on October 15, 2014 and was used to evaluate whether a PPS would be awarded the second half of their design grant award, to be disbursed on November 3, 2014.The original questionnaires and responses are now publically available here by clicking on the appropriate PPS links.
