Dear Mr. Helgerson:

This letter is to inform you that the Centers for Medicare & Medicaid Services (CMS) has received New York´s Hospital-Medical Home final evaluation report required under the Special Terms and Conditions for the state´s Partnership Plan section 1115 demonstration (Project No. 11-W-00114/2).

CMS accepts New York´s report that found that all participating sites experienced some level of achievement and transformation. Notable improvements include reaching National Committee for Quality Assurance (NCQA) Level 3 status, restructuring training programs to increase residency time in ambulatory settings, and several improvements in inpatient quality and safety. CMS also notes several policy recommendations that are listed in the report. We look forward to relevant updates from New York on these recommendations and how it may be reflected in the state´s ongoing delivery system reform efforts.
