Single Source Procurement: Community Health Worker Expansion Program - Contract Extensions

Pursuant to New York State Finance Law § 163.10(b), The New York State Department of Health is presenting the following summary of relevant circumstances, and material and substantial reasons why a competitive procurement was not feasible. The New York State Department of Health, Community Health Worker Expansion Program has made a determination to issue a Single Source Procurement to 23 eligible grantees to support ongoing currently funded projects.

The goal of the Community Health Worker Expansion Program (CHWE) is to improve maternal and infant health outcomes, and reduce disparities in those outcomes, for high-need, low income, Medicaid-eligible women, and their families. Through collective efforts with community partners, the program addresses key barriers that impact maternal and infant health outcomes, and support families to effectively access continuous and coordinated healthcare, and other services and supports in their community, including home visiting.

Under the supervision of a licensed professional, CHWs assist high-need, Medicaid-eligible women and families with access to needed services, and provide: outreach; screening/assessments; referral to services and follow-up; information on relevant health topics, including but not limited to prenatal care, childbirth education and support, breastfeeding, and assisting in the development of birth plans; advocacy, and supporting increased health literacy; and enhanced social supports, such as promoting collaborative childcare and support networks.
