Single Source Procurement: Rural Health Care Access Development Plan – Contract Assignment
Pursuant to New York State Finance Law § 163.10(b), The New York State Department of Health is presenting the following summary of relevant circumstances, and material and substantial reasons why a competitive procurement was not feasible. The New York State Department of Health, Rural Health Care Access Development Plan Program has made a determination to issue a Single Source Procurement to one eligible grantee to support ongoing currently funded projects.
In 2019, Herkimer County Rural Health Network (HCHN) was awarded a contract for the ‘Rural Health Network Development Program’ (RHNDP). HCHN provides services to the Herkimer County on:
Chronic disease - the network has developed a patient-centered care model where the patient is recognized as the expert of their health. This empowers the patient to share their information with providers, engage in planning and become an active leader if their own health by promoting prevention. The network also provides classes to help with chronic conditions, such diabetes self-management, smoking cessation classes and chronic disease self-management.
Dental health - the network distributes over 2,000 dental hygiene kits to children in daycare, early childhood education and K-12 schools. The kits include toothbrush, toothpaste, and educational materials.
Transportation barriers – the network provides support to two services, Community Transportation Services (CTS, serving Old Forge) and RIDE (southern region of county) for program enhancement and service reach expansion to healthcare providers.
In 2020, the RHNDP received a cut in the 2020-21 state enacted budget due to the COVID19 pandemic. The loss of revenue had the HCHN reaching out to the county for assistance. Herkimer County agreed to take over the financial portion of the contract for the period of January 1, 2021 – December 31, 2023.
