Single Source Procurement: Open Doors Transition Centers Extension

Pursuant to New York State Finance Law § 163.10(b), The New York State Department of Health is presenting the following summary of relevant circumstances, and material and substantial reasons why a competitive procurement was not feasible. The New York State Department of Health Office of Health Insurance Programs Division of Employee and Program Support Program has made a determination to issue a Single Source Procurement to one eligible grantee to support ongoing currently funded projects. New York Association on Independent Living is currently funded to administer the Open Doors Transition Centers. Open Door Transition Centers identify potential participants in nursing facilities who are interested in transitioning to the community, educating them on their return to community options, and facilitating successful transitions to their community of choice. Transition Centers currently facilitate community transitions that otherwise could not have occurred or would likely have resulted in re-institutionalization. Additional components and supplemental funding made available to align with a previously let competitive procurement and cannot stand alone.
