Single Source Procurement: Official New York State Prescription Form

Pursuant to New York State Finance Law § 163.10(b), the New York State Department of Health is presenting the following summary of relevant circumstances, material, and substantial reasons why a competitive procurement was not feasible. The New York State Department of Health, Office of Primary Care and Health Systems Management, Bureau of Narcotic Enforcement has made a determination to extend its contract with the incumbent contractor on a single source basis for up to fifteen months to avoid an interruption of supply while it competitively procures the replacement contract.
