Single Source Procurement: Alzheimer's Disease Community Assistance Program

The purpose of this single source procurement is to extend contract number C028497 from March 31, 2018 to June 30, 2018 with the NYS Coalition of Alzheimer's Association Chapters (AA, NYS Coalition) for the provision of services under the Alzheimer's Disease Community Assistance Program (AlzCAP). Historically, AlzCAP was a sole/single contract with the AA, NYS Coalition. Due to an expansion of funding for Alzheimer's disease and other dementias (AD/D) support services throughout NYS, delivery of the AlzCAP program is now subject to a competitive procurement process. Given the length of the procurement and approval process, this request to extend the contract for three months will ensure there are no gaps in support services.

The Alzheimer's Disease Program (ADP), located within the Bureau of Community Integration and Alzheimers Disease, is part of the Division of Long Term Care in the Office of Health Insurance Programs. The ADP is responsible for implementing initiatives from a significant annual appropriation to support people with Alzheimer's disease and other dementias (AD/D) and their caregivers. This appropriation represents the single largest state investment in these services in history and is aimed to make a significant difference in the lives of the targeted population. The initiatives are based on evidence that demonstrates that providing an array of caregiver services in the community helps avoid unnecessary hospitalizations and emergency room visits, delays nursing home placement, and improves caregiver burden and mental health outcomes. The AlzCAP program in one of several initiatives that is primarily funded under the appropriation and is participating in a comprehensive evaluation to measure the impact of caregiver support services on caregivers throughout New York State (NYS).

The initiative supports Public Health Law, Article 20-A, Section 2005 and 2006 which expresses intent to encourage and foster home and community-based assistance through an existing network of not-for-profit organizations whose primary mission is to respond to the needs of NYS's Alzheimer's disease community. The New York State Department of Health currently has a Single/Sole Source Contract (C028497) with the AA, NYS Coalition to implement and oversee the Alzheimer's Community Assistance Program (PHL Article 20-A.)

Through this contract, the AA, NYS Coalition funds local Alzheimer's Association Chapters and CaringKind in New York City (NYC) (the former NYC Alzheimer's Association) to meet contract deliverables. These organizations, in turn, provide support services to persons with AD/D and their nonprofessional caregivers throughout NYS. These services currently include, but are not limited to, care planning and consultation, caregiver training, support groups, a 24-Hour Helpline, community awareness, and increased training for important constituencies. All services reflect evidenced-based practice with the goal of postponing and/or avoiding nursing home placement.

This is a three month extension of the contract(s) under the same terms, conditions, and pricing authorized under the existing contract(s). The single source extension period will allow for the continuation of critical services pending outcome of a new competitive procurement to obtain these services.
