Single Source Procurement: Alzheimer’s Disease Program - Alzheimer’s Disease Caregiver Support Initiative

Pursuant to New York State Finance Law § 163.10(b), The New York State Department of Health is presenting the following summary of relevant circumstances, and material and substantial reasons why a competitive procurement was not feasible. The New York State Department of Health Alzheimer’s Disease Program has made a determination to issue a Single Source Procurement to 10 eligible grantees to support ongoing currently funded projects.

The funds through this will provide provides funding and support to health and community-based organizations that work to decrease the burden of Alzheimer’s disease and other dementias (AD/D) on individuals living with AD/D, their caregivers and family members throughout NYS. The ADP initiatives are based on evidence that demonstrates that providing an array of caregiver services in the community helps reduce unnecessary hospitalizations and emergency room visits, delays nursing home placement, and improves caregiver burden and mental health outcomes. Another important goal is to promote early diagnosis of AD/D

The caregiver support services includes care consultations, support groups, education and training programs, respite, and at least one of the following additional allowable services: access services, caregiver companions, care support teams, caregiver wellness programs, joint enrichment programs, safety services and technology-based services.
