Long Term Home Health Care (Lombardi) Program Complaint Process
Do you have a complaint regarding the Long Term Home Health Care Program (LTHHCP)?
Complaints about specific LTHHCP services should primarily be made directly to your LTHHCP provider agency. The contact numbers are listed on the "Consumer Contact Information" sheet given to each participant at the time of waiver enrollment, at each reassessment, and whenever requested.
The New York State Department of Health (NYSDOH) also operates a Home Health Care Hotline that affords individuals the opportunity to register grievances or complaints about the home care services. To file a complaint, call the toll free hotline at 1-800-628-5972.
All complaints will be forwarded to the NYSDOH Regional Office in which the service provider is located. NYSDOH staff will acknowledge the complaint by letter. The final determination will be communicated in writing to the person initiating the complaint, as appropriate and allowed by HIPAA confidentiality regulations.
Complaints about the LTHHCP waiver may also be filed at the Local Department of Social Services district office. The contact numbers, by County, are listed below.
