Assisted Living Program Application

The New York State Department of Health (DOH), Center for Health Care Provider Services and Oversight, Division of Adult Care Facilities/Assisted Living Surveillance announces the Assisted Living Program 3400 Initiative. This initiative is made available under Section 461-1 of the Social Services Law (SSL), authorizing the addition of 6,000 Assisted Living Program (ALP) beds pursuant to a seven-year plan ending January 1, 2017. An Assisted Living Program provides supportive housing and home care services to individuals who are medically eligible for placement in a nursing facility, but whose needs can be met in a less restrictive and lower cost residential setting.

Interested applicants must submit the Opportunity for Development (OFD) proposal by 3:00 p.m. June 22, 2015. This form and instructions, as well as other relevant information, can be accessed below:
