Dear Colleague Letter - Announcement of New Statewide Central Register Database Check Form (LDSS-3370) for Early Intervention Providers

August 31, 2009

Dear Colleague:

On August 4, 2009, The Office of Children & Family Services (OCFS) released an Informational Letter (INF) that affects all early intervention provider agencies and municipalities. The INF provides information about changes that have been made to the Statewide Central Register Database Check Form (LDSS-3370), which is completed by applicants who are screened by the New York State Statewide Central Register (SCR) to determine whether they are the subjects of indicated reports of child abuse or maltreatment on file with the SCR. The INF describes new requirements for completing this form and describes how to obtain copies of the revised version of LDSS-3370.

As stated in the INF, provider agencies should discard all copies of previous versions of the Statewide Central Register Database Check Form (LDSS-3370), and obtain copies of the newly revised form to give to persons who must be screened.

The Informational Letter (INF) is available on the OCFS web page at http://www.ocfs.state.ny.us/main/policies/external/#INF

Please share this information with all providers of early intervention services.

Sincerely,

Margaret Adeigbo
Provider Approval Unit Manager
