The Long Term Home Health Care Program Reference Manual was developed to assist local district and provider agency staff. It provides a clear explanation of the Long Term Home Health Care Program (LTHHCP), along with practical guidelines regarding its usage, so that they can directly apply the information to their work of serving clients.

However, while this manual is designed to be versatile and user-friendly, it does not substitute, or in any way replace, State law, regulations, and policy directives governing the LTHHCP. The LTHHCP Reference Manual is intended to be a handy guide, but it is not meant to provide the same comprehensive treatment of the subject as the previously mentioned governing authorities. All applicable statutes, regulations, and policies are listed at the end of each chapter and should be referenced as needed for more detailed information.

The Long Term Home Health Care Program Reference Manual was developed by Susan Barone, under the auspices of the Center for Development of Human Services. Funding for this project was provided by the New York State Department of Health through the New York State Office of Children and Family Services (NYS OCFS) Bureau of Training.
