1. How do I create an account to access the Public Authenticated NYSE-CON System?

3. Where can I get the necessary CON schedules to submit with my application?

The latest schedules are available online for one to download and complete. The Public Authenticated NYSE-CON system will indicate which schedules one needs to submit based on the type of application being submitted. Follow the steps below to access the schedules:

  1. Go to this link http://www.health.ny.gov/facilities/cons/nysecon/
  2. Under the CON Applications bullet click on the Forms and Schedules link

4. How can I get rid of the pop up message asking if I would like to access a non-trusted site while navigating in Public View?

If you are using Internet Explorer 7 or 8, follow the following steps:

  1. Select the Tools/Internet Options and then the Security tab
  2. Select the Trusted Sites zone
  3. Select the Sites button
  4. Enter https://apps.health.ny.gov in the trusted sites field and select the Add button
  5. Close the first window and then select OK to return to the web page

5. How do I add multiple instances of a document in the Correspondence section?

When Schedule 2 is selected as the document type you will see the following message 'A unique description is required for this document.', please use the following unique information in the description in this order: Last Name, First Name Schedule # section letter

For example:

  1. Smith, Pat Schedule 2
  2. Jones, Pat Schedule 2a
  3. Smith, Ann Schedule 2b
  4. Smith, John Schedule 2c

This will create a separate instance that can be updated individually as needed throughout the process.

6. How do I reset my password?

  1. Click this link to go to the NY.gov sign in page https://my.ny.gov/
  2. Click on “Forgot your Password?”
  3. Follow the instructions on the screen.
  4. You may reset your password by answering security questions or by requesting an email message with instructions.

7. How do I get access to my project(s) if I can no longer log in?

  1. Create a new User ID
  2. Send an email to nysecon@health.ny.gov requesting access to your project. Please include the following information: facility name, submission #, and contact information for the new user.
  3. You will be contacted if additional information is required.
